File a Civil Rights/Discrimination Complaint

When to file your civil rights complaint online

Use the online civil rights form to file complaints related to:

  • Discrimination in employment
If you believe your employer discriminated against you because of your membership in a protected class or retaliated against you because you filed a complaint, assisted with someone else's complaint, or opposed discrimination at work.
  • Discrimination in housing
If you believe you were discriminated against in the rental, leasing, or purchasing of property because of your membership in a protected class.
  • Family Medical Leave Act
If your employer refused a request to take leave under the FMLA or took a negative action against you because you took FMLA.

Civil rights complaints related to Public Accommodations law, the Healthcare retaliation laws, the State Whistleblower law, or others must be filed using a paper form.
