DVR On-the-Job Training (OJT) Checklist

Note: This document is specific to the DVR OJT process and does not apply to DVR Student On-The-Job Training (S-OJT).

• DVR staff discusses OJT service with Consumer. Describe the opportunity for DVR to provide a training incentive (50 percent of wages reimbursed) to an employer who offers Consumer a permanent position in accordance with their Individualized Plan for Employment (IPE) goal. Explain that by participating in OJT, Consumer's association with DVR will be disclosed to the employer.
• DVR staff and Consumer include OJT service in Consumer's IPE.
• When applicable, DVR staff and Consumer notify Service Provider that Consumer is interested in pursuing OJT with an employer.
• When applicable, Service Provider may offer OJT to businesses when providing Job Development Services. DVR staff must follow-up to discuss with prospective employers.
• DVR staff discusses OJT with Employer before Consumer hire is finalized. All five questions below must be answered "Yes" before proceeding with the OJT:
  • Is the training for a job that aligns with the DVR Consumer's IPE goal?
  • Are wage and benefits compensated at or above the minimum wage, and not less than the customary wage and level of benefits paid by the employer to others performing similar work?
  • Do work hours meet the DVR Consumer’s needs, whether part-time or full-time up to 40 hours per week?
  • Is the DVR Consumer considered a permanent employee?
  • Does the employer agree to maintain the Consumer's employment and pay wages and benefits in full following the OJT period?
• DVR staff checks to see if Employer is in STAR and IRIS.
• DVR staff contacts Employer to discuss the hire, goal of the OJT, and next steps.
• DVR staff provides DVR OJT Guide for Employers to Employer.
• DVR staff works with Employer to complete New Vendor forms if not in STAR:
  • New Supplier Form DOA-6460 can be requested through your DVR Staff contact.
  • Request for Taxpayer Identification Number and Certification IRS W-9 Form
• DVR staff works with Employer to complete the OJT Hiring Initiative Agreement and Training Plan.
• DVR staff explains Monthly Invoice/Progress Report to Employer and provides electronic/paper copies and contact information for a local DVR staff person to submit reports.
• DVR staff, Consumer, and Employer sign agreement.
• DVR staff sends the completed and signed OJT Hiring Initiative Agreement to DVR Scanning.
• DVR staff explains to Employer and Consumer that Consumer must review and sign-off on Employer's progress report before it can be submitted to DVR.
• DVR staff completes OJT purchase authorization using this language, "This authorization is for 50 percent of the wages associated with the paid training required for the permanent hire of the individual named above. A total of XX hours per week at an hourly rate of $XX is approved for this individual's training."
• DVR staff approves and completes the OJT purchase order coded 058. Note: Do not send purchase order to Employer, as the purchase order does not trigger payment, instead the monthly progress report serves as the invoice.
• If Consumer chooses retention services through a Service Provider, DVR staff authorizes the purchase order for 90 calendar day retention services to Service Provider.
• DVR staff reviews monthly progress reports to assess training progress.
• Upon receipt of the 30-Day Invoice and Progress Report, DVR staff date stamps and forwards copies to CCP at DWD MB DVR Vendor Invoices for payment processing.
• DVR staff sends monthly progress reports to DVR Scanning.
• DVR staff completes receiving on hours/dates of work reflected in the Progress Report/Invoice.
• CCP processes and schedules payments to Employer within one (1) business day of receipt of each monthly progress report.
• Service Provider provides follow-along services for 90 calendar days.
• DVR staff provides follow-along services for Consumer for an additional 90 calendar days after completion of OJT.
