Subject: Due May 17th: 2024-2026 DVR Service Agreement

Dear Service Provider,

This communication has only been sent to the Main Contact and contains important information about the 2024-2026 DVR Service Agreement (Effective 07/01/2024 through 06/30/2026).

Please share with your staff that are responsible for managing your agency's DVR Service Provider Portal Account and DVR Service Agreement.

Your current DVR Service Agreement (Agreement) will expire on June 30, 2024.

If you plan to renew your Agreement for the 2024-2026 term (July 1, 2024 through June 30, 2026), please review the Agreement Renewal Resources at 2024 Service Agreement - Online Account Updates.

Agreements for the 2024-2026 term must be submitted to DVR by Friday, May 17, 2024.

Agreements submitted to DVR after May 17th may not be reviewed until August 1, 2024. New DVR referrals may be suspended until your Agreement is approved.

Please Note: You must complete all personnel and/or subcontractor portal profile updates on the Personnel Page before going to the Agreement Page and selecting, Add New Service Agreement. If you start a new agreement before completing all required updates on the Personnel Page, DVR will have to deny the agreement in order to allow you to make the required personnel and/or subcontractor profile updates.

Portal profile updates include:

2024 IT Security Awareness Training (completion date must be 03/08/2024 or after)

Ethics Training (if completion date exceeds five (5) years)

Criminal Background Check Date

Required for all personnel and subcontractors
Completion date must be within six (6) months preceding the signing of the 2024 Agreement
