Subject: Due May 17th: 2024-2026 DVR Service Agreement
Dear Service Provider,
This communication has only been sent to the Main Contact and contains important information about the 2024-2026 DVR Service Agreement (Effective 07/01/2024 through 06/30/2026).
Please share with your staff that are responsible for managing your agency's DVR Service Provider Portal Account and DVR Service Agreement.
Agreements for the 2024-2026 term must be submitted to DVR by Friday, May 17, 2024.
Agreements submitted to DVR after May 17th may not be reviewed until August 1, 2024. New DVR referrals may be suspended until your Agreement is approved.
Please Note: You must complete all personnel and/or subcontractor portal profile updates on the Personnel Page before going to the Agreement Page and selecting, Add New Service Agreement. If you start a new agreement before completing all required updates on the Personnel Page, DVR will have to deny the agreement in order to allow you to make the required personnel and/or subcontractor profile updates.
We hope that you were able to attend one of the live sessions discussing the updated personnel/location functionality in the portal since the sessions covered how the updates will impact your Agreement/portal account.
