Dear Service Provider,

The following communication has only been sent to the designated main contact in your DVR portal account. Please share with your staff as you see appropriate.

Effective: July 1, 2024

This Replaces:

Information related to the 2022-2024 Report/Forms and Technical Specifications. DVR will continue to accept previous report versions received through August 31, 2024.

A new Statewide Service Fee Schedule has been published, but no fee increases are included at this time.

Why is this change being made?

To share changes and updates to Reports/Forms and Technical Specifications that have been implemented. The changes have been designed to streamline reporting content, enhance the ease and quality of reporting, and provide additional clarity on service delivery requirements.

The updates have been shared with DVR staff during a May Sync Session. Important information and copies of the recording and materials from that session can be found below.

What is the Change?

Report Use & Instructions: (updated/revised to be posted 6/28/24):
1. Job Development Plan, DVR-17036-E
2. Job Development Monthly Report, DVR-18028-E
  * Previously part of the Job Development Plan
3. Transportation Plan, DVR-19827-E
  * Optional report. Previously part of the Job Development Plan
4. Retention: Form, DVR-17038-E
5. IPS Career Profile Report, DVR-18018-E
6. Job/Task Analysis and Systematic Instruction Report, DVR-18212-E
Technical Specifications: Tech Specs are now updated to align with the changes to the reports/forms.

Important information

All services completed prior through June 30, 2024, should complete, and submit the previous published reports.
All services completed after July 1, 2024, should complete and submit the new published reports.
The 2022-2024 Published reporting templates will no longer available after September 1, 2024.
