SUBJECT: Fax number change for optical imaging system facsimile equipment

The fax number for the Worker’s Compensation Division’s optical imaging facsimile server has been changed to (608) 260-2503. The imaging fax server accepts correspondence, documents and reports associated with a worker’s compensation claim via electronic fax. Information sent via this number will be received directly into the division’s optical imaging system. The department is also equipped to transmit claim information directly from the optical imaging system to internal and external clients via fax.

Please use this number to submit claim documents listed on the fax cover sheet. (Click on " W " icon at top of page to download in Microsoft Word 6.0.) Unless it is necessary to meet promptness standards or you are having mail service problems, please do not routinely fax the following documents:

* WKC-12 – The Employer’s First Report of Injury or Disease
* WKC-13 – Supplementary Report on Accidents and Industrial Diseases
* WKC-13A – Wage Information
* Any documents relating to claims pending a hearing or legal decision.

When faxing claim information to the division, please include the division’s worker’s compensation claim number, claimant’s social security number, claimant’s name and date of injury.

Please use the fax cover sheet (above) when transmitting claim information. This cover sheet will insure the information you transmit is correctly processed and filed. Always include your telephone number and return fax number on the cover sheet.

The Worker’s Compensation Division now has only one telephone fax number for receipt and/or transmission of information. The fax telephone number is: (608) 260-2503. We encourage you to use this feature as an alternative to mailing correspondence to the division.
