Description: This form may be used by employers to report changes in name, address, telephone or business operations, or to report a Federal Employer Identification Number (FEIN), to update their Unemployment Insurance (UI) account.

Comments: Employers without online access may use this account change form. This form is an electronic Microsoft Word template that can be filled out on your computer (if you have Microsoft Word). If you do not have Microsoft Word, we are providing a PDF which you can print and complete by hand. Completed forms may be submitted by email, fax or mail.

Employers with online access may report changes (e.g., name, address, phone, operations) to your UI account in the UI Employer Portal.
