Description:  This form is used by DVR staff to determine eligibility for post-secondary funding from DVR.  The form will also calculate what funds may be available to the DVR student.

Comments:  This form can be used to collect a signature from a consumer for the release of related information to the school.  It can also be used to calculate training grant funds for DVR students attending FAFSA eligible programs not included in the online portal.  Do NOT use this form for programs that are not FAFSA eligible.
