Supported Employment Coordination Planning

The Wisconsin Department of Workforce Development (DWD), Division of Vocational Rehabilitation (DVR) has signed a Memorandum of Agreement (MOA) with the Wisconsin Department of Health Services (DHS) - Division of Medicaid Services (DMS). The agreement identifies a shared vision of collaboration, cooperation, and communication to provide service standards for common customers to obtain and maintain Competitive Integrated Employment (CIE).

This agreement describes three points of review of the Supported Employment Coordination Plan in DVR's process for common customers. Supported Employment services can be provided to DVR consumers determined eligible in Category 1. The Supported Employment Coordination Plan contains documentation of agreement and plan coordination among the Supported Employment Team of individuals assisting each common customer. The purpose of three points of contact in the DVR process is to engage with the Supported Employment Team to identify resources, provide supports, and to discuss successful coordination and transition of services for CIE outcomes for the common customer.

The coordinated plan section included in the DVR reports is intended to provide an opportunity for members of the Supported Employment Team to reach agreement about the steps of the consumer's employment process. Any respective agency plans, including Individualized Plans for Employment (IPEs), Member Centered Plans (MCPs), and Individual Support and Service Plans (ISSPs) should be discussed and the services contained in those plans should be coordinated to benefit the consumer and to further their CIE goals.

Supported Employment Team communication should be documented and include an agreement for planned services, employment, payment, timing, and intended outcomes for the common customer. A meeting should be conducted with all members of the Supported Employment Team invited to develop relationships and discuss the common customer, CIE outcomes, and process. The Supported Employment Team can choose to meet remotely or over the phone.

A review of the Supported Employment Coordination Plan should take place at the following three points in the DVR process:

  1. DVR Post Discovery/Career Profile meeting/assessment
  2. DVR 60-day on-the-job meeting
  3. Before Transition to Long-term Support

DVR has updated reporting to include documentation of the agreement of the Supported Employment Team, at the appropriate times, as identified in the MOA. Reports are completed by DVR service providers and sent to DVR upon the completion of the associated service.

If a member of the Supported Employment Team is not present to sign the report, initials and a date can be entered reflecting their verbal agreement, or an email from the person can be sent along with the report following the meeting to show agreement.

The DVR service provider and DVR staff should be prepared to share copies of the final approved reports listed below with the Long-Term Care provider upon request after the specific meeting or after agreement is communicated. A Release of Information (ROI) form is not required.
