Transition Services for Students with Disabilities

DVR works with students with disabilities who are transitioning from high school to employment and training. DVR works with the Department of Public Instruction (DPI) and the Department of Health Services (DHS) to help students have a smooth transition from school to work and community services.

Statewide Transition Action and Resource Team (START): DVR's team of local resource staff who can answer your questions about transition services

Transition Action Guide (TAG): Helps students, parents, teachers and others to understand what they can expect, and their responsibilities during transition from high school to work and community services

DVR Services to Students and Families

Students, and their families, can expect the following from DVR staff:

Offer you information about DVR services, including pre-employment transition services

Share information with you about DVR eligibility requirements

Attend your IEP meetings, when invited, at least two years before graduation

Provide you information about applying to DVR and determine your eligibility for DVR services

If you are eligible for DVR services, help you develop an Individualized Plan for Employment (IPE) and coordinate services with your school and DHS

Monitor progress toward your employment goal

DVR Services to Schools

Schools can expect the following from DVR staff:

Provide information and outreach services to students with disabilities

Assist the student in identifying post-school employment goals

Offer a DVR application and determine eligibility for all student referrals

Coordinate services, including IPE, Individualized Education Program (IEP), and pre-employment transition services

Attend IEP meetings, when invited, and monitor progress toward the student's employment goal
