On your DWD letter, locate the Employer # and Mailing ID next to the QR Code in the upper right-hand corner. Enter those values and click Search. The form presented will ask to verify your business information and answer questions from your letter. After submitting the form, you will receive an acknowledgement number for future reference.

Each letter is allowed one online form submission. Additional documentation in PDF format may be uploaded for the Worker's Compensation investigator from the Upload Documents menu.
