From this portal, you can fill out forms you received, upload documentation, and view past submissions/uploads. For each menu option you will need the Employer # and Mailing ID listed on your letter next to the QR Code.

• Search allows you to find your form and fill it out.
• Upload Documents allows you to upload PDF files for review. Files may be uploaded even if you do not complete the form online.
• Submission History allows you to review your responses or uploaded documents.
• Help provides answers to common questions related to the portal as well as answers to common insurance coverage requirements.
