Dear Service Provider,

You are receiving this notice because you are identified as the main point of contact for the Division of Vocational Rehabilitation (DVR) Service Provider Agreement. Pursuant to Section 10.1. Amendment of the Terms and Conditions, DVR is providing 30 days written notice to amend Section 4.9. Transporting DVR Consumers, effective July 1, 2023.

Section 4.9, Transporting DVR Consumers has been revised to read as follows:
Transportation is not an included service component of this DVR Service Provider Agreement. However, the Service Provider may elect to provide transportation to DVR consumers in connection with providing the included service components of this Agreement at the Service Provider's own cost, convenience, and risk. If a Service Provider elects to provide this type of transportation to DVR consumers, the Service Provider shall maintain a commercial or business automobile liability insurance policy against any claim(s) which might occur in providing transportation to DVR consumers. Minimum coverage shall be a combined single limit of one million dollars ($1,000,000). The Service Provider may not charge DVR for providing transportation to DVR consumers under this Agreement.

This Agreement does not prohibit a Service Provider with a transportation division, from providing otherwise allowable transportation services under a separate agreement or authorization with DVR.

If a Service Provider elects to provide unfunded transportation to a DVR consumer connected with an authorized service identified in the DVR Service Agreement e.g., Job Preparation, Development, Hire, and Retention Services, they must check "Yes" on the Service Provider Details Page in the DVR Service Provider Portal prior to transporting a DVR consumer. By checking "Yes," the Service Provider is indicating to DWD that it has read, understood, and is agreeing to comply with all the terms and conditions set forth in the revised Section 4.9, Transporting DVR Consumers, effective July 1, 2023. The "indicator" is tentatively scheduled to be available in the portal on June 26th. An announcement will be sent to all DVR Service Provider main contacts once the update is completed. No action is required if a Service Provider elects not to provide the transportation services as identified in Section 4.9.
