DVR is sharing this grant announcement from the Wisconsin Department of Health Services with all DVR Service Providers. The grants aim to enact innovative changes that will benefit participants, families, and the long-term care workforce in Wisconsin. The initiative aims to positively impact people receiving Home and Community-Based Services (HCBS) from providers regionally, locally, and in urban and rural communities throughout the state. This includes improvements in the delivery of employment services. A total of $15 million is available.

Application Period to Open Soon for $15 Million in Grants Supporting Quality and Innovation in Wisconsin Medicaid Home and Community-Based Services
The Wisconsin Department of Health Services (DHS) is launching the second round of Home and Community-Based Services (HCBS) Grants Supporting Quality and Innovation, funded through the American Rescue Plan Act (ARPA). These grants offer an exciting opportunity for entities active in the state’s Medicaid HCBS system to apply for $25,000 to $1 million to enact innovative changes that will benefit participants, families, and the long-term care workforce in Wisconsin. The initiative aims to positively impact people receiving HCBS from providers regionally, locally, and in urban and rural communities throughout the state. A total of $15 million is available.
