What is this Replacing?

This is new information to accompany changes to DVR Supported Employment reporting implemented in July 2020.

Why is this change being made?

To share with DVR staff and partners who serve mutual consumers of planned coordination documentation during supported employment services.

The Wisconsin Department of Workforce Development (DWD), Division of Vocational Rehabilitation (DVR) has signed a Memorandum of Agreement (MOA) with the Wisconsin Department of Health Services (DHS) - Division of Medicaid Services (DMS). Within the agreement three points of coordination were identified. The purpose of three points of contact in the DVR process is to engage with the Supported Employment Team to identify resources, provide supports, and to discuss successful coordination and transition of services for CIE outcomes for the common customer.

A review of the Supported Employment Coordination Plan should take place at the following three points in the DVR process:

  1. DVR Post Discovery/Career Profile meeting/assessment
  2. DVR 60-day on-the-job meeting
  3. Before Transition to Long-term Support

DVR reporting includes documentation of service coordination.
