Communicate with focus person about their goals for their life and help them identify what is important to be happy, healthy, and safe. Identify what gives meaning to the focus person s life. Ensure focus person s voice is heard when making plans/decisions about their lives. Participate in identifying goals and next steps that are in line with the focus person s goals for their life. Involve and engage focus person in planning for and carrying out activities across their day, week, and month. Contribute to documenting information about things tried and what was learned about new activities implemented with focus person. Implement Positive and Proactive Behavior Management Strategies.
