This software only training is designed to support individuals with a base knowledge and skill of a Community Health Worker. Care Coordination System License for trainees to provide assessment tools that assure that at-risk individuals are identified and enrolled in programs that effectively meet their health and social needs. CCS helps coordinate care in order to reduce duplication of services (and costs) while encouraging a team approach to health and healthcare that improves communication between providers and human services systems and enhances the efficiency of providing appropriate care. This training provides an introduction into Health careers. The following are the core competencies software trained Community Health Workers (CHW) gain from training: Analyzing Data:Use data effectively to improve outcomes, upload referrals and resources and analyze results and includes familiarity with the data available. Navigating Database Software:Learn how to use built-in forms and reports in a database. Write queries and reports using available tools; copying data into Excel or other formats for further analysis. Data Integrity:Understand data definitions, program guidelines, and sources of data. Review data and working with colleagues to make sure that data is accurate. Become aware of data gaps when analyzing and using the data. Database Design/Planning:Understand database design concepts. Understand the benefits/limits of various types of databases. Ability to participate in short-term and long-term planning about database projects analyze various type of data
For this training UniteMKE partner's with the WI Community Health Worker network, WI DHS and a host of other employer partners to connect with partner employers to support the CHW placement.
Ongoing one on one support for trainee pre and post training.

PLEASE NOTE:

If there are fewer than eight enrollments in the training program, only the "Enrollments" number will be displayed; all other data is hidden in order to help protect the students' identities. The "Employment Outcomes" are calculated using information from Wisconsin's Unemployment Insurance Wage database. Most employers are required to report certain employment information to the State of Wisconsin, including quarterly wage information for their employees. Students whose information could not be verified in this database are excluded from these outcomes. If a "0" shows for any of these outcomes, it means that none of the students' information could be verified.

MAKING SENSE OF THE EMPLOYMENT OUTCOMES:

"Average quarterly wages" means the total wages earned in a three-month period by all students who completed the program ("completers"), divided by the total number of completers.

"Median quarterly wages" means all the completers' quarterly wages are placed in order from lowest to highest and then we identify the student's wages in the middle. This information is provided because averages can be greatly influenced by really high and low wage earners.

To estimate monthly wages, divide the quarterly wages by three.
