File a Petition for Review (appeal) of an Administrative Law Judge's decision on-line - -

In addition to the other methods by which a petition for commission review (appeal) of an ALJ's decision may be filed, petitions for review of administrative law judges' decisions in  Unemployment Insurance or Worker's Compensation cases may now also be filed on-line, by using a form at this website.

To file a petition for commission review of an administrative law judge's decision in an Unemployment Insurance or Worker's Compensation case, use one of the following forms:

Please Note:  This form may NOT be used to appeal from an Initial Determination issued by a department deputy.  This form is ONLY for filing an appeal of an administrative law judge's  Appeal Tribunal Decision.

redball.jpg (817 bytes) Equal Rights cases -- Please note:  Electronic filing of a petition for review is NOT available to appeal decisions of administrative law judges of the Equal Rights Division in Fair Employment and Public Accommodation cases.  Petitions for review in such cases must be filed with the Equal Rights Division.
