Instructions for the Prevailing Wage On-Line Survey

Please read these instructions before clicking on the links below.

Mail completed survey on a 3 ½ inch floppy disk or compact disc along with your signed survey form to:

Labor Standards Bureau

PO Box 7887

Madison  WI  53707-7887

Or e-mail completed survey to:

pwsurvey@dwd.wisconsin.gov

and fax your signed survey form with the statement “Survey data submitted via e-mail” to:

Instructions for sending your survey via e-mail:
After saving your survey records in the application, you are now going to want to save a file to your computer to send.  When sending us your file, DO NOT send us the file named pw_on_line_survey.  That file is the survey application and we are unable to view any of your records. When you go to save your records, the application automatically prompts you to insert a 3 ½ inch floppy disk into your “A” drive.  Steps to follow:

If you have an “A” drive:
1.
 Insert a disk and hit OK.  You may keep the disk for your records and disregard saving another file for DWD since you will be e-mailing your file.  The file that needs to be attached to your e-mail is now on your “A” drive with the file name being your control numberà “####.txt”
2.  Open your e-mail application and compose a new e-mail.  To insert the correct file, there will either be a paper clip at the top of your e-mail or you may go to “Insert”, then “File”.  Look in your “A” drive and attach the text file mentioned previously.

If you do NOT have an “A” drive:
1. 
To save a copy for your records:  Instead of saving your records to the “A” drive you are going to need to open the “Edit Survey Records” screen.  Highlight all of your records then copy and paste them into a new Excel file.  You now can save that file under any name that you choose for your records.

 To e-mail your copy:  Open your e-mail application and compose a new e-mail.  To insert the correct file, there will either be a paper clip at the top of your e-mail or you may go to “Insert”, then “File”.  Look in your “C” drive, usually called “Local Disk.”  The file that needs to be attached to your e-mail is in your “C” drive with the file name being your control numberà “####.txt” Insert that file and send. 

Instructions for sending your survey via mail on diskette:
When sending us your file, DO NOT send us the file named pw_on_line_survey.  That file is the survey application and we are unable to view any of your records.  When you go to save your records the application automatically prompts you to insert a 3 ½ inch floppy disk into your “A” drive.  You will need 2 disks, one for your records, one to send.  Insert a disk and click on “Save Employer File Copy” and hit OK.  Insert second disk and click on “Save File for DWD.” 

These instructions tell you how to save the on-line survey application to your local personal computer based on the web browser you use.

Internet Explorer Users:
In order to use the Prevailing Wage On-Line Survey application, you must right click on the "Equal Rights – Prevailing Wage On-Line Survey" link. You will see a menu of choices. Choose "Save Target As". Then pick a location on your C:\ drive. Once it is done saving, you must "Close> the save window. It now resides on your C:\ drive and is named: "PW On-Line Survey.xls".

All Other Web Browser Users:

In order to use the Prevailing Wage On-Line Survey application, you need to save it. You should save it to your C:\ drive. It will then reside on your C:\ drive and is named: "PW On-Line Survey.xls".

When you have completed the annual prevailing wage survey using the web based survey method you will need to copy the information to a diskette, sign the tear out form from the booklet and mail both to the department.
