Prevailing Wage On-Line Survey Application

Thank you for considering using the Prevailing Wage On-Line Survey application. It is the hope of the Equal Rights Division that you will find this application a great improvement over the paper and pencil/pen method. You will need to have an employer specific "control" and "form" number to use the web based annual prevailing wage survey method. These two items will be printed on the annual prevailing wage survey booklet. Additional instructions and other reporting methods are also available in the survey booklet. This application has been developed using Microsoft Excel. In order to use the Prevailing Wage On-Line Survey application you will need to have Excel loaded on your personal computer. To be able to use this application, you will need to make sure that your security level in Excel is not set on "High". To do this: Open Excel, at the top of the screen click on “Tools,” “Macro,” and finally “Security.”
