These instructions are applicable only to union contractors. If a contractor has employees who work under a collective bargaining agreement and their hourly basic rate and fringe benefit rate equals or exceeds the project determination prevailing wage rate, then besides reporting the employer’s UI Number, Prevailing Wage Project Determination Number, Job Code, and Employee Under Union Contract (Y/N), the contractor/employer must identify the applicable Union local and provide a copy of the collective bargaining agreement (CBA).
The Department maintains a database of CBAs. If the CBA or wage allocation sheet you wish to report is already a part of the Department’s database, then you need not provide a copy of the CBA. However, you must correctly identify the applicable CBA in your certified payroll report. For this reason, the Department has created the List of Union Short Names, Locals & Collective Bargaining Agreements for you to accurately select the applicable CBA. You must insert the correct Union Short Name in the field called “Name of Union” and the correct Local Short Name in the field called “Local Union Number.”

On the list, find the Union Name corresponding to the union you seek. Next find the Local Number and/or Collective Bargaining Agreement Name to which you are signatory. Write down the applicable Union Short Name and Local Short Name appearing on the list. This is the information you will need to insert in the certified payroll reporting fields called “Name of Union” and “Local Union Number.”
If the Department has a copy of the collective bargaining agreement (CBA) to which you are signatory, then the column on the List entitled Union Contract will say CLICK HERE. You will want to “click” to confirm that the linked document is the CBA and/or wage allocation sheet that lists the wage rates you paid your employees on the reported prevailing wage project. Assuming that you are satisfied with the displayed CBA and/or wage allocation sheet, then insert the Union Short Name and Local Short Name in your certified payroll report fields called “Name of Union” and “Local Union Number.” You should receive an “Upload Successful” message after uploading your certified payroll data to this website.
If you find the CBA to which you are signatory but the Department does not have a copy of the CBA document or applicable wage allocation sheet that lists the wage rates you paid your employees on the reported prevailing wage project, then the column on the list entitled Union Contract will say NONE and you will need to upload a PDF (& only a PDF format) of the applicable CBA and/or wage allocation sheet to the certified payroll report website. and select Upload a Collective Bargaining Agreement and follow the instructions. After completing this process and when you complete your certified payroll report, you must insert the correct Union Short Name in the field called “Name of Union” and the correct Local Short Name in the field called “Local Union Number.” You will not successfully upload your certified payroll data if there is no CBA document for the Union Short Name and Local Short Name you list in your certified payroll report.

and select Upload a Collective Bargaining Agreement and then select Add a New Union or Local Number and follow the instructions. Once you have completed the steps in the instructions, it may take up to five business days to receive back an email from the Department giving you the Department-determined Union Short Name and Local Short Name for the union name, local number and/or CBA name which you wish to add to the database. In your certified payroll report, you must insert the correct Union Short Name in the field called “Name of Union” and the correct Local Short Name in the field called “Local Union Number.” You will not successfully upload your certified payroll data if you have not inserted a Department-determined Union Short Name and Local Short Name in your certified payroll report.
