Weekly Work Search Log

Document Number:  EB-12

Description:  This form is used to record a claimant's weekly search for work while filing for Extended Benefits (EB).  Do not use this work search log if you are not an EB claimant.  If you are not an EB claimant, sending in this form might prevent payment of benefits.  EB claimants receive a notice of EB eligibility.

Comments:  This form must be completed on a weekly basis while a claimant is collecting Extended Benefits (EB).  The form must be sent weekly to the following address or fax number below:

The internet work search form is available as part of the internet weekly claim.  The claimant must make the weekly claim on the internet in order to enter the weekly work search record on the internet.  The internet weekly claim can be filed on the UI Weekly Claim Certification website.
