Description: This form is used by employers (construction contractors) who wish to secure a project-specific certification that confirms their compliance with or exemption from the apprentice utilization requirements that are associated with state funded construction projects. This form is for use by contractors or subcontractors who do not meet the Annual Certification requirements. It can be completed and submitted to DWD by the project's lowest eligible bidder and its intended subcontractors.
