Employee Count Report to DWD/DHFS

Description:  This form is completed by county Human (Social) Services Departments to report their employee counts to the state four times each year in March, June, September and December.  The data is used to allocate the Shared Cost reported on form FIS-13591-E.  This form is provided to both DWD and DHFS.
