To file a Discrimination Complaint, fill out the form on the following pages. You will be asked to provide information concerning the following items:
  • Complainant
  • Contact
  • Respondent
  • Employment Status with the Respondent
  • Reasons for the Discrimination
  • A Written Statement detailing the Discrimination

Filing with Other Agencies

Before you begin, please indicate whether or not you have previously filed a complaint for this matter with another agency. Our agency has a work-sharing agreement with the U.S. Equal Employment Opportunity Commission (EEOC) for complaints alleging discrimination on the basis of age, race, creed, color, sex, national origin, or disability. We will handle the complaint if it is filed with us first and we have jurisdiction. We will also co-file the complaint with the EEOC if it meets the criteria for filing under federal law.
