No. We encourage all employers to report wage data via automated methods, and especially those reporting 25 or more employees, those reporting data for multiple account numbers, and service agencies and accountants.
No. If you would like us to verify that the format of your data is acceptable, you can send us a file clearly labeled as test data, but we do not require a test file. You can send live data the first time you submit your wage report on automated media.
We suggest using the first six digits of your UI account number, or an abbreviation of your company name.
Yes. You can submit data for multiple employer accounts in a single file, as long as the file is prepared using one of our three approved data formats, and each employer is identified by their correct UI Account Number. If you submit data for multiple accounts, please include a Transmitter Report (UCB-7822), listing each UI Account Number, and number of records submitted for each account.
Include your company name, address, contact person, phone number, UI account number, quarter year of data reported, and number of employees reported. After you report wage data to UI on a diskette for the first time, we will mail a postage paid label and a preprinted diskette label to you every quarter. When information on the label changes, please cross out and print the correct information on the label and we will update our records.
Yes. Report total gross wages paid to each employee in the calendar quarter on your quarterly wage report, regardless of whether they have met the payroll exclusion for wages paid in 2009.
No, we do not accept substitute forms. We use an optical scanner to scan our paper reports and the data must line up and be in our exact format. If you have a computer to print substitute forms, you probably have the capability to create a file and submit your wage report via electronic or magnetic media, which is preferable to paper reporting.
No. You need to create your own text file using one of the three acceptable data formats, and save it as an ASCII text file using the .txt file extension. Once you have the text file created, you can either send us the file on diskette or CD-ROM, or upload the file via the Internet.
You can file your Contribution Report on paper and mail it with payment to: Unemployment Insurance, P. O. Box 78960, Milwaukee, WI 53278-0960. Or you can file your Contribution Report via the Internet and and make payment via Electronic Funds Transfer (EFT).
As of 3rd quarter 2007, if you report wages for 50 or more employees, your Contribution Report must be filed online.
As of 3rd quarter 2008, if you report wages for 25 or more employees, your Contribution Report must be filed online.
